Homeless after leaving hospital

Informing the hospital

If you have nowhere to live after you are discharged from hospital, you should tell the hospital nursing staff as soon as possible, so that they know that you will be homeless when you are discharged. They will ask the hospital discharge team to help if they can.

The discharge team will refer you to the Housing Options Team at Ipswich Borough Council or your local council if you from outside Ipswich. They will assess your situation and you may be offered temporary accommodation while your application is assessed.

Your allocated Housing Options Officer will develop a Personal Housing Plan with you and help advise, and assist you in finding longer term accommodation.
